What is Tolvaptan used for?
Tolvaptan is a selective vasopressin V2-receptor antagonist used to treat clinically significant hypervolemic and euvolemic hyponatremia [serum sodium <125 mEq/L or less marked hyponatremia that is symptomatic and has resisted correction with fluid restriction], including patients with heart failure, cirrhosis, and Syndrome of Inappropriate Antidiuretic Hormone (SIADH).

HOW SUPPLIED/STORAGE AND HANDLING:
Store at 25 °C (77 °F), excursions permitted between 15 °C and 30 °C (59°F to 86°F). Keep the tablets out of reach of children.
TOLVAPTAN SIDE EFFECTS
The most commonly reported Tolvaptan side effects include: thirst, asthenia, dry mouth, constipation, pollakiuria or polyuria, and hyperglycemia.
TOLVAPTAN DOSAGE
The usual starting dose for tolvaptan tablets is 15 mg administered once daily without regard to meals. The dose can be increased to 30 mg once daily, after at least 24 hours, to a maximum of 60 mg once daily, as needed to achieve the desired level of serum sodium. During the initiation and titration, frequently monitor for changes in the serum electrolytes and volume. Do not skip/miss the doses of Tolvaptan. If you miss a dose, take it as soon as you remember. If it is near the time of the next dose, skip the missed dose. Just take the next dose at around your regular time. Do not take a couple of doses at the same time. In case you take excess pills of tolvaptan 15 mg, call your health specialist right away.
WARNINGS AND PRECAUTIONS
- Following discontinuation from tolvaptan tablets 15 mg, patients should resume the fluid restriction and should be monitored for changes in the serum sodium and volume status.
- Patients treated with tolvaptan should be monitored in order to assess serum sodium concentrations and neurologic status, especially during initiation and following titration.
- Discontinue or interrupt tolvaptan uses in those patients who develop too rapid a rise in serum sodium, and consider administration of hypotonic fluid.
- Fluid restriction during the initial 24 hours of therapy with tolvaptan may increase the likelihood of overly-rapid correction of serum sodium, and should generally be avoided.
- Fluid restriction during therapy with tolvaptan samsca may increase the risk of dehydration and hypovolemia. Patients with tolvaptan should continue ingestion of fluid in response to thirst.
- Respective patients should avoid conceiving a baby and should not breastfeed an infant if they are taking treatment with tolvaptan.
- Patients should not drink grapefruit juice while on treatment with tab tolvaptan 15 mg. This could cause you to have excess tolvaptan in your blood.
